Our Place is a Class AA, Community Based Residential Facility (CBRF), located in Green Bay, Wisconsin (Brown County). The facility has a warm and friendly environment, serving up to 18 adult residents with mental or emotional disorders, who have difficulty caring for themselves on their own. Services are designed to reduce or stabilize psychiatric symptoms, increase psychosocial functioning and community integration, and improve the quality of life for those in our care.

Services include:

  -  Client assessment prior to admission to identify specific needs and abilities.

  -  Development of an individualized service plan for each resident with family and guardian involvement encouraged.

  -  Ongoing case management services provided on-site by trained social workers.

  -  Supervision of resident activities 24 hours a day.

  -  On-going communication with residents regarding applicable community resources, services, and programs.

  -  Scheduled weekly leisure activities to promote interaction and camaraderie among residents.

  -  Encouragement of family visits and assistance with arranging visits as needed.

  -  Arrangement for transportation of residents to medical appointments, work, training programs, religious services, and community activities  

     as needed.

  -  Annual health assessments if the resident sees his/her physician less than twice per year.

  -  Monitoring and administration of all client medication. Residents with the ability to self-administer their medication are encouraged to 

     demonstrate this skill.

  -  Instruction, as needed, in areas related to self-care such as eating, personal hygiene, dressing, grooming, etc.

  -  Independent living skills instruction including money management, food preparation, laundry, cleaning, use of public transportation, 

      shopping and vocational skills.

  -  Opportunities to improve social skills, communication techniques and strengthen personal relationships through a combination of group 

      and individual activities.

  -  Additional mental health services from Brown County or private clinics in the community as needed.
